Applicants over 75: we do not currently arrange new private cover for over-75s, but we can still help any family members aged 75 or under.
We do not currently work with insurers that offer new private health insurance policies for applicants over 75. If you are over 75, you may need to rely on public healthcare, S1 entitlement where applicable, existing cover, or private pay-as-you-go healthcare. If a spouse, partner or family member is aged 75 or under, we may still be able to review private health insurance options for that person. See residency over 70.
New private health insurance is difficult to arrange after 75, which is why our position is that we do not offer new private policies for over-75 applicants. The routes that may be available include public healthcare where you are covered through an accepted route; S1 entitlement for eligible pensioners; continuing an existing private policy, which may be possible depending on the insurer's terms; and private pay-as-you-go healthcare for specific treatment. Which applies depends on your nationality, residency route and circumstances — confirm with a qualified immigration professional.
Where a residency application involves an over-75 applicant alongside a spouse, partner or family member aged 75 or under, each person's age is assessed individually — see cover for couples. We may still be able to review private health insurance options for the under-75 person while the over-75 applicant explores public healthcare, S1 or existing cover. At renewal, an over-75 resident with existing cover should check whether that policy continues. A common mistake is assuming a new private policy can be bought after 75 — plan healthcare routes before moving.

An existing private policy may be able to continue depending on the insurer's terms, even though new policies are not available over 75. Check with your current insurer.
Some eligible pensioners may use an S1 for public healthcare in Spain. Whether it applies depends on your circumstances, so check before relying on it.
Where you are covered through an accepted public healthcare route, that may serve as your healthcare evidence. Check what your residency route requires.
Each applicant's age is assessed individually. A spouse or partner aged 75 or under may still be able to arrange private health insurance, subject to eligibility, while the over-75 applicant explores other routes.
