Private health insurance availability changes significantly at older ages. For Spanish visa and residency applications, this can be especially important because the policy may need to provide clear documentation, healthcare cover in Spain and specific wording required by the consulate or immigration office.
For applicants over 75, the issue is not only price. It may be that new private visa-suitable cover is not available through the insurers we work with.
If you are over 75, you may need to check whether one of these routes applies: S1 entitlement where applicable; public healthcare access in Spain; existing Spanish private health insurance that can continue; existing international health insurance; healthcare access through a family member route; healthcare evidence accepted for a renewal; specialist residency advice based on your visa route; or private pay-as-you-go healthcare, where appropriate.
This should be checked before assuming that a new private health insurance policy can be arranged.
Some retirees may have S1 entitlement depending on nationality, pension status and personal circumstances. Where available and accepted for the route, an S1 may provide healthcare access in Spain without needing to buy a new private health insurance policy.
However, S1 entitlement is not automatic for everyone, and the evidence required can vary by route and authority.
Some applicants may already have or later obtain public healthcare access in Spain through a recognised route. This may be relevant for renewals, long-term residents, family-member routes, S1 holders, social security contributors, and applicants already registered in the Spanish healthcare system.
If you are applying for a visa from outside Spain, check carefully whether public healthcare evidence is available and accepted for your route.
Some applicants over 75 may already have Spanish private health insurance or international private health insurance. This is different from starting a new policy.
If you already have cover, check whether the policy is still active; whether it can continue after age 75; whether it covers healthcare in Spain; whether it can provide a certificate; whether it has copayments, deductibles or reimbursement terms; whether it covers the required period; whether proof of payment is available; and whether the consulate or immigration office accepts it.

For the applicant over 75, a different healthcare route may be needed, such as S1 entitlement, public healthcare access, existing cover or specialist residency advice.
The Non-Lucrative Visa is a common route for retirees, but over-75 health insurance can be difficult because new private visa-suitable policies may not be available through the insurers we work with. If you are over 75 and considering the NLV, check your healthcare route before preparing the rest of the application.
Possible routes to review may include S1 entitlement where applicable, existing private cover, public healthcare evidence if available, specialist residency advice, and whether another visa or residency route is more appropriate.

Renewal applicants over 75 may be in a different position from first-time applicants. If you are already in Spain and renewing a visa, residency or TIE, you should check whether your current insurance can continue; whether you now have public healthcare access; whether S1 entitlement applies; whether the renewal office accepts your healthcare evidence; whether an updated certificate is available; and whether proof of payment is needed.
Family reunification cases involving applicants over 75 should be reviewed carefully because the healthcare evidence may depend on the sponsor, the route, the applicant's status and whether private insurance is required. If private insurance is needed, new cover over 75 may not be available through our service.
Travel insurance is usually not the clearest option for Spanish visa or residency applications because it is designed for temporary trips, emergencies, cancellations and luggage. For over-75 applicants, travel insurance may also not solve the main issue if the visa route requires healthcare evidence equivalent to private health insurance or public healthcare access.
Some applicants over 75 may already hold international health insurance. This should be checked carefully before relying on it for a Spanish visa or residency file. It may create issues if it is reimbursement-based; has deductibles or excesses; does not clearly cover Spain; cannot provide suitable certificate wording; does not cover the required period; lacks proof of payment; or is not accepted by the relevant authority.
If you are aged 70–75, you may still be able to request private health insurance options for Spain, subject to eligibility. Do not wait until after turning 75 if you need private visa-suitable cover for a Spanish visa or residency application.
If your visa appointment is close and the applicant is over 75, it is important to act quickly, but also realistically. We do not currently work with insurers offering new private visa-suitable policies for applicants over 75.
For urgent review, gather applicant age, visa route, application country or consulate, deadline, existing insurance documents, S1 or public healthcare evidence if any, renewal documents if already in Spain, and any consulate or immigration-office request.
We prefer to be clear from the start. If we cannot arrange new private visa-suitable health insurance for an over-75 applicant, we do not want you to waste time completing a normal quote process.
