S1 vs Private Health Insurance for NLV Spain

Some NLV applicants may have healthcare evidence through S1 or another recognised public healthcare route, while others may need private health insurance. We help applicants review private options where private cover is required.

S1 vs private health insurance: the basic difference

Some applicants may have access to public healthcare through S1 or another recognised route. Others may need private health insurance to support the Non-Lucrative Visa application. This should be checked before buying private insurance or assuming S1 applies. If private insurance is needed, we help applicants review visa-suitable options for Spain. For the main NLV page, see NLV health insurance.

Who's who

Who may have S1, and who may still need private cover

An S1 is generally linked to state pension or certain benefits in your home country, and lets eligible people access public healthcare in Spain. Whether you qualify depends on your circumstances, so confirm it through the relevant authority rather than assuming. Those retiring before state pension age usually do not yet have an S1 and often need private cover for the NLV file. For UK retirees specifically, see NLV health insurance for UK retirees and retiring to Spain on the NLV.

Couples, age and private cover where needed

In a couple it's common for one partner to hold an S1 while the other arranges private health insurance. If an S1 is accepted for your route, private cover may not be required — but check what your route and consulate expect. Where private cover is needed, options can be reviewed by age, including over 65; note we do not work with insurers offering new policies for applicants over 75. A common mistake is assuming S1 applies without confirming it.

Common situations

Common S1 and private insurance situations for NLV applicants

Both applicants have S1

If both applicants have valid S1 entitlement and the route accepts it, private health insurance may not be needed for the visa file. The key point is making sure the documentation is accepted for your application route.

One applicant has S1, the other does not

This is common for couples where one person is at state pension age and the other is younger. The person without S1 may still need private health insurance for the NLV application.

No S1 yet

If you have not received S1 or are not yet eligible, private health insurance is often the clearer route for the NLV application.

Over 75 and no S1

This can be difficult because we do not currently work with insurers offering new private visa-suitable policies for applicants over 75. Applicants in this position may need to explore public healthcare access, S1 where applicable, existing cover or specialist residency advice.

Answers

S1 vs private FAQs

Can S1 be used for the Spanish NLV?+

Some eligible state pensioners may be able to use an S1 to access public healthcare in Spain instead of private insurance. Whether you qualify depends on your circumstances, so check before relying on it.

Who may be eligible for S1?+

S1 is generally linked to state pension or certain benefits in your home country. Eligibility depends on your situation, so you should confirm it through the relevant authority.

What if I am retiring before state pension age?+

If you retire before state pension age you may not yet qualify for an S1, so private health insurance is often needed for the NLV until another route applies.

What if only one spouse has S1?+

This is common. The S1 holder may use public healthcare while the other partner arranges private health insurance for the visa file.

Do I still need private insurance if I have S1?+

If an S1 is accepted for your route, private insurance may not be required, but some applicants still choose private cover. Check what your route and consulate expect.

What if I do not have S1 yet?+

What if I am over 75?+

Can you help if private insurance is needed?+

Yes, where private cover is needed we help applicants review visa-suitable private health insurance options for Spain, including certificate-ready cover.
