An EU citizen often has their own healthcare route — through work, social security, or an S1 — and in some cases that route covers the non-EU spouse as a dependant. Where it does not, the spouse usually needs separate healthcare evidence, which is commonly private health insurance with a certificate. Whether you apply together or join later can affect the documents, and cover can usually be arranged before the spouse has an NIE or TIE, using passport details first.
The certificate usually needs to be in the spouse's own name showing healthcare cover in Spain — see residency health insurance. Existing international or travel insurance is often not the clearest option for living in Spain. Couples sometimes have different healthcare routes (one public, one private). Applicants over 60, 65 and 70 can usually request options subject to eligibility; we do not work with insurers offering new policies for a spouse over 75. A common mistake is assuming the EU citizen's cover automatically extends to the spouse without checking.
