Family members of EU citizens may need healthcare evidence when applying for residency or family member documentation in Spain. Depending on the situation, this may be public healthcare access, S1, an employment or self-employment healthcare route, or private health insurance. If private health insurance is needed, we help applicants review options for Spain, including certificate-ready documentation where required. See health insurance for Spanish residency.
An EU citizen often has their own healthcare route through work, S1 or other entitlement, while a non-EU spouse or partner may need separate healthcare evidence. Children and dependants included in a family application usually need evidence too, which may be covered through the family's route or may need private insurance with a certificate. Parents and older dependants are assessed individually, and where private cover is required, the over-75 limit applies.
Where the EU citizen works or is self-employed in Spain, public healthcare access may come through social security; eligible pensioners may use an S1, which can extend to dependants in some cases. Where neither applies, private health insurance is often used. Cover can usually be arranged before you have an NIE, using passport details first. Applicants over 60, 65 and 70 can usually request options subject to eligibility; we do not work with insurers offering new policies for applicants over 75. A common mistake is assuming one person's route automatically covers the whole family.
Family members of EU citizens may need to show healthcare evidence when applying for residency or family member documentation in Spain. Whether private insurance is needed depends on the situation.
A non-EU spouse may need healthcare evidence, which can be private health insurance or another accepted route such as public healthcare through the EU citizen's work. Check your circumstances.

Some eligible pensioners may use an S1 for public healthcare, which can extend to dependants in some cases. Whether it applies depends on circumstances, so check before relying on it.

Private health insurance can often be arranged before you have an NIE, usually using passport details, with the NIE added later where needed.
We do not currently work with insurers offering new private visa-suitable policies for applicants over 75. An over-75 family member may need to explore public healthcare access, S1 where applicable, existing cover or specialist residency advice.
