Applying for a self-employed visa or work route in Spain? We help applicants review healthcare evidence and private health insurance options where required, including cover during the application stage.

Self-employed visa and work-route applicants should check what healthcare evidence is required for their specific application. Some applicants may become covered through social security once registered in Spain as autónomo. Others may need private health insurance during the application stage or where private cover is requested. If private health insurance is needed, we help applicants review options for Spain, including certificate-ready documentation where required. See health insurance for Spanish residency.
Once registered as autónomo and paying social security, public healthcare access usually follows — so for many self-employed residents, private insurance is mainly relevant to the period before social security is active, or where the application specifically asks for private cover. Whether you apply from the Spanish consulate or are already in Spain changing status can affect what's needed. Freelancers on the digital-nomad route are different — see DNV freelancers. We don't claim every self-employed route requires private insurance — it depends on your route and stage.
Where private cover is used, the certificate and proof of payment should show healthcare cover in Spain, and cover can usually be arranged before you have an NIE. Dependants included usually need their own evidence. Existing international or travel insurance should be checked for clear Spain cover. Applicants over 60–70 can usually request options subject to eligibility; we do not work with insurers offering new policies for applicants over 75. A common mistake is assuming social security covers you from day one of the application.

In some cases yes, private health insurance is used during the application stage or before social security is active. Check what your specific route requires.
International insurance may help in some cases, but it should be checked for clear Spain cover and a suitable certificate before relying on it for the application.
Dependants included in the application usually need healthcare evidence too, which may be private health insurance with a certificate for each person.
Private health insurance can often be arranged before you have an NIE, usually using passport details, with the NIE added later where needed.
We do not currently work with insurers offering new private visa-suitable policies for applicants over 75. Over-75 applicants may need to explore public healthcare access, existing cover or specialist residency advice.
