A common misunderstanding

Can EU citizens use an EHIC for Spanish residency?

Not as a general rule for long-term residence. An EHIC is mainly for medically necessary public healthcare during temporary stays, and it is not designed as a long-term private health insurance replacement. It also shouldn’t be confused with an S1.

Context does matter, though. For students, some Spanish guidance treats an EHIC differently where it is valid for the full residence period and covers necessary treatment — so the exact situation counts. But for long-term residence as a worker, pensioner or financially independent resident, check whether you need Spanish public healthcare proof, S1 evidence or private insurance rather than relying on the EHIC.

Some can. An S1 can transfer healthcare responsibility from one EU country to another in certain cases, so some EU pensioners or people with qualifying entitlement may be able to use one to access Spanish public healthcare. But not every EU citizen automatically qualifies, and an S1 needs to be applied for and evidenced properly. If you’re relying on an S1, obtain official proof before cancelling any private cover — and if it doesn’t apply, private insurance may be needed instead.

Can EU citizens arrange health insurance before getting an NIE?

Usually, yes. A policy and certificate can normally be arranged using your passport or national ID details before you have an NIE — which can help before EU registration or administrative appointments. The certificate can show your passport or ID details, and the insurer may be able to update them later depending on their process. Family members can often be quoted using passport details too. For EU retirees, the practical order is: check S1 first if you may qualify. If S1 or public healthcare proof is not available, private cover may be needed. Age and medical history matter for what’s available, and you shouldn’t cancel your public entitlement or any private cover without checking your Spanish position first.
