What health insurance do you need for Spain's Digital Nomad Visa?

Health insurance for Spain's Digital Nomad Visa depends on your work situation and the healthcare evidence you can provide. Some DNV applicants may rely on social security, an A1 certificate, employer healthcare evidence or another recognised route. Other applicants may need private health insurance for Spain.

Where private health insurance is needed, the policy should be selected around the visa requirement, applicant age, family members, certificate wording and start date. We help Digital Nomad Visa applicants review Spain-focused private health insurance options, including no-copay and certificate-ready cover where required. If you already know private cover is needed and want to compare options, see our guide to the best health insurance for Spain’s Digital Nomad Visa.

DNV vs NLV

Why Digital Nomad Visa health insurance is different

Digital Nomad Visa health insurance is not always as simple as "buy a private policy". The correct route can depend on whether you are an employee, freelancer, company owner or contractor, and whether you have social security coverage that can be used for the application.

That said, many DNV applicants still need or prefer private health insurance because they do not have recognised social security evidence; employer cover does not clearly apply in Spain; family members are not covered; international insurance is not suitable; they need certificate-ready private cover; they want private healthcare access in Spain; or their current certificate has been questioned.

What's usually needed

Typical Digital Nomad Visa health insurance requirements

Where private health insurance is required for a DNV application, it should usually be selected around the healthcare evidence requested for the visa. Depending on the applicant and application route, the policy may need:

Private healthcare cover in Spain

Certificate or proof of cover

No copayments where required

No deductibles or excesses where required

Policy start date aligned with the visa or arrival date

Cover for the required period

Cover for spouse, partner, children or dependants where included

Proof of payment where requested

Insurer authorised to operate in Spain where required

Employees

Employees of a foreign company

If you are employed by a foreign company, your healthcare route may depend on your social security position and whether employer cover clearly applies while you live in Spain. Employer insurance is not automatically suitable for the Digital Nomad Visa, so it may need checking before you rely on it.

Freelancers

Freelancers and self-employed applicants

Freelancers and self-employed applicants may need to show healthcare evidence that matches their DNV route. Where private health insurance is needed, the policy should usually be selected around no-copay wording, certificate wording, start date and any family members included.

Company owners

Company owners

Company owners should check whether their healthcare evidence is linked to employment, company ownership, social security contributions or private cover. Private health insurance may be needed where your existing evidence does not clearly support the application.

Families

Families and dependants

Family applications need careful checking because the main applicant's healthcare evidence may not automatically cover a spouse, partner, children or dependants. Where private cover is needed for family members, each applicant should be named correctly and covered for the required period.

No-copay

Does DNV health insurance need to be no-copay?

Where private health insurance is required for a Digital Nomad Visa application, no-copay cover may be requested or preferred depending on the application instructions. If your consulate, immigration office or adviser mentions sin copago, no copayments, no deductibles or no excesses, tell us before arranging cover.

See no-copay health insurance for Spanish visas.

Certificate

Do you need a certificate for DNV health insurance?

If private health insurance is being used as healthcare evidence for your Digital Nomad Visa, you may need a certificate, policy document or proof of cover. A useful certificate should usually show the applicant name, insurer name, policy start date, cover period, healthcare cover in Spain, dependants where included, no-copay wording where required, and proof of payment where requested.

See the DNV health insurance certificate guide.

International cover?

Can international insurance be used for the Digital Nomad Visa?

International health insurance may provide good medical cover, but it is not automatically suitable for Spain's Digital Nomad Visa. It can create issues if it is reimbursement-based; has deductibles or excesses; does not clearly cover residence in Spain; does not name dependants; cannot provide suitable certificate wording; or is not accepted as healthcare evidence for your application route.

See international vs Spanish visa health insurance.

Travel cover?

Can travel insurance be used for the Digital Nomad Visa?

Travel insurance is usually not the right option for a Digital Nomad Visa because it is designed for temporary trips, emergencies, cancellation and luggage, not living and working remotely from Spain. If private health insurance is needed, a Spain-focused private policy is usually clearer.

See travel insurance vs visa health insurance.

Before NIE

Can you get DNV health insurance before having an NIE?

Yes, many applicants arrange private health insurance before having an NIE because it may be needed for the visa or residence application. Depending on insurer rules, passport details may be used first and the NIE added later where needed.

See health insurance without an NIE.

Start date

What start date should DNV health insurance have?

The correct start date can depend on your application route, planned arrival date, appointment date, submission timing and whether private insurance is being used as healthcare evidence. Check the application date; planned arrival date; expected residence period; family member dates; policy certificate wording; proof of payment timing; and whether cover is needed immediately.

See the visa health insurance start date guide.

Renewal

Health insurance for Digital Nomad Visa renewal

If you are renewing your Digital Nomad Visa or residence authorisation, you may need to show continued healthcare access. This may involve private health insurance; Spanish social security evidence; employer evidence; family member cover; updated certificate documentation; proof that cover remains active; and policy start and end dates.

See DNV health insurance renewal.

Urgent

Need DNV health insurance urgently?

Urgent DNV enquiries are common when applicants realise close to submission that their employer insurance, travel insurance or international policy may not be suitable. For urgent help, include your work situation; application country or immigration office; appointment or submission deadline; number of applicants; applicant dates of birth; whether family members are applying; preferred start date; whether you have an NIE; whether private cover is required; whether no-copay cover is required; and whether your current certificate has been questioned.
