US health insurance can be excellent within the United States, but it may not provide clear healthcare cover or visa documentation for Spain. Spanish visa applications usually focus on whether the insurance provides suitable cover for Spain and whether the certificate matches the application requirements. If your US plan is emergency-only abroad, reimbursement-based, has high deductibles, or cannot issue the right certificate, Spanish private health insurance may be a clearer route.

Many US plans are domestic, with emergency-only or out-of-network cover abroad and reimbursement after you pay. For a Spanish residence visa, the documentation should clearly show ongoing healthcare cover in Spain for the visa period. Deductibles, copays and policy limits can also be an issue where no copayments are required. Medicare generally does not cover healthcare outside the United States, so it is usually not suitable. NLV, DNV, Student Visa and US employer plans

For the NLV, US cover should be checked carefully as the visa commonly requires private cover for Spain with no copayments where required. The same documentation points apply for the DNV and Student Visa.
